Oral, Dental, and Maxillofacial Surgery: Diagnosis and Treatment Processes

Oral, dental, and maxillofacial surgery is a medical discipline concerned with the diagnosis and surgical treatment of diseases, traumas, and anatomical disorders occurring in a broad area encompassing the face, jaws, oral cavity, and teeth. As a maxillofacial surgeon, the cases we encounter in our clinical practice range from a simple tooth extraction to anatomical jaw reconstructions. In order for our patients to better understand their treatment processes, we will address the fundamental scope and workings of our field from a clinical perspective.

Which Diseases and Treatments Does Maxillofacial Surgery Cover?

The treatment of patients who apply to our clinic requires mastery of surgical anatomy. The main areas of intervention in oral and maxillofacial surgery are as follows:

  • Impacted Tooth Operations: Surgical extraction or orthodontic exposure of wisdom teeth and canines that have not erupted due to their anatomical position.

  • Cyst and Tumor Surgery: Surgical removal and biopsy of pathological formations (cysts, tumors) that develop in oral tissues or within the jawbone.

  • Dental Implantology: Placement of intraosseous titanium screws in place of missing teeth. In cases where bone volume is insufficient, bone grafting (bone powder augmentation) and sinus lift procedures.

  • Jaw and Facial Trauma: Surgical repair of jaw and facial bone fractures resulting from falls, accidents, or impacts.

  • Orthognathic Surgery: Surgical correction of situations in which the lower and upper jaws are skeletally incompatible with each other or with the face as a whole.

  • Temporomandibular Joint (TMJ) Disorders: Treatment of issues such as jaw joint pain, joint disc displacement, and jaw locking.

Preoperative Clinical Evaluation

Before a surgical intervention, it is essential to obtain the patient’s full medical history (past illnesses, medications used, allergies). In patients with systemic conditions (diabetes, hypertension, heart diseases), consultation is carried out with the physicians following the patient. In radiological evaluation, in cases where standard panoramic X-rays are insufficient, 3-dimensional dental volumetric tomography (CBCT) is used to calculate bone thickness, the position of nerve canals, and sinus cavities to the millimeter. This clinical planning minimizes the risk of complications during the operation.

Surgical Procedure and Recovery Period

Surgical procedures are performed under local anesthesia, conscious sedation, or general anesthesia depending on the patient’s medical condition and the extent of the procedure. The primary aim is to optimize tissue healing by minimizing surgical trauma.

The basic medical rules our patients should observe after the operation are as follows:

  • During the first 24 hours, intermittent cold compresses should be applied to the area to control the development of edema (swelling).

  • To protect the surgical site from infection, the prescribed antibiotics, analgesics (pain relievers), and antiseptic mouthwashes should be used in the dose and duration specified by the physician.

  • During the tissue healing process, soft, warm, and non-irritating foods should be consumed for the first few days.

  • To prevent disruption of the healthy blood clot formed in the area, spitting, rinsing, or using straws that create pressure should not be done during the first 24 hours.

Oral and maxillofacial surgery is a specialized field that requires strict compliance with sterilization rules. In cases of loss of jaw and intraoral function that you may experience, being examined by a maxillofacial surgery specialist for the correct diagnosis and treatment protocol is of great importance for the integrity of your tissues.
